Verifying Insurance Coverage

To verify your insurance coverage:

Contact your insurance provider by calling the customer service phone number on the back of your insurance card. There may be a specific number for mental health/behavioral health services. Asking the following questions is recommended:

  • Do my benefits include coverage for behavioral health services?
  • Is Dr. Parker an in-network or out-of-network provider for my plan?
  • If in-network: What is my co-pay for an outpatient office visit? Do I have co-insurance?
  • If out-of-network: Do I have out-of-network benefits? What is the reimbursement rate to see a licensed clinical psychologist?
  • Do I have a deductible? (If yes: What amount has been reached for the year?)
  • Is a referral or pre-authorization required for routine outpatient mental health services?
  • How many sessions per year does my plan cover?
